To enroll in First Steps Preschool, the following must be on file in the office on or before the first day of attendance: 

          Completed Enrollment form

          Non-refundable registration fee

Immunization records must include the child’s name, date of birth, parent's address, dates of each immunization, and the name of the health care provider.

Mississippi requires Form 121, approved by the Mississippi State Department of Health. The 121 form must remain up to date in your child’s file.

If First Steps Preschool is charged a fee due to an outdated 121 form, you will be charged the fee.

         Complete Doctor’s Release (in the event of allergies) *
